Admissions
Before admittance can be granted into an assisted living home, the prospective Resident’s physician must complete a Physician’s Assessment Form. In addition to this form,
Ma Maison also requires that a Self-Care Assessment is completed. An assessment by the assisted living manager is also completed upon meeting with your loved one.
Once the Physician’s Assessment and Assisted Living Manager Assessment are completed, a Level of Care is determined. Each question on these assessments is allotted certain point values. Based on the total scoring, a Level of Care is determined.
